The preliminary investigation has determined the driver of a Chevrolet Express cargo van crossed over the centerline of the road and struck a Jeep SUV in the opposite lane head-on.
The driver of the cargo van, Michael J. Simons-Iocona, age 28, of Williamstown, NY, was transported to Upstate Medical Center, where he died from his injuries. The driver of the Jeep, Tammy L. Piscitelli, age 54, of Lysander, NY, suffered serious but non-life-threatening injuries and was transported to Upstate Medical Center for treatment. A 3-year-old passenger in the Jeep was also transported to Upstate Medical Center for evaluation.
The investigation into the crash is ongoing; no tickets have been issued.
