On September 29, 2025, the Rhinebeck Bureau of Criminal Investigation (BCI) received a report of alleged physical abuse by a staff member of Devereux Advanced Behavioral Health, located at 40 Devereux Way in the town of Red Hook, New York.
The investigation conducted by the Bureau of Criminal Investigation determined that staff member Samuel I. Ezechukwu, 49, of Red Hook, New York, struck a resident, the victim, in the back and held the resident’s head down while transporting facility residents.
As a result of the investigation, on September 30, 2025, Ezechukwu was arrested and charged with Endangering the Welfare of an Incompetent or Physically Disabled Person in the First Degree, a class E felony.
Ezechukwu was released on an appearance ticket returnable to the Town of Rhinebeck Court on October 8, 2025, at 5:00 p.m.
