Under the CLS's former accreditation program, certain information related to the analysis of a test item was not required to be documented in the associated laboratory report as long as the information was recorded elsewhere in the technical record. This exemption is no longer allowed under our current accreditation program without a written agreement with your agency. Due to current limitations in the CLS's information management system, some reports may not include the information, as outlined below.
Per our customer agreement with your agency, the following information may not be included on case reports, but exists elsewhere in the technical record.
Biological Science Casework
- Identification of the method used
- The date(s) of performance of the laboratory activity
- The date of receipt of the test item(s)
Drug Chemistry
- The date(s) of performance of the laboratory activity
- The date of receipt of the test item(s)
- The date of sampling
Firearms
- Identification of the method used
- The date(s) of performance of the laboratory activity
- Identification of the database(s) used
- The date of receipt of the test item(s)
Fire Debris
- The date(s) of performance of the laboratory activity
Forensic Identification Unit (Latent Print/ Friction Ridge)
- The date of receipt of the test item(s)
Toxicology reports contain all required information.
Please note that reported results relate only to the items tested or sampled, as identified in the test report.

If there are questions regarding the minor portion of DNA mixtures in Biological Science DNA Case Reports issued between October 21, 2021, and January 28, 2022, please contact the laboratory.
