
Trooper Gary E. Kubasiak was shot and killed August 30, 1982, after responding to the scene of a reported domestic dispute.
The subject, who had a history of incarceration for mental health reasons, had threatened his mother who ran to a neighbor's house and called for help. Trooper Kubasiak and his assigned K9, Donovan, were recalled to duty to assist an on-scene detail with the arrest of the barricaded subject.
Trooper Kubasiak, who knew the subject personally, entered the kitchen of the man's residence to talk him into surrendering. Without warning, the subject shot and fatally wounded Trooper Kubasiak in the chest, before he could defend himself or command his dog to attack.
Trooper Kubasiak was a nine-year veteran of the State Police and had worked for seven years out of the Gowanda Satellite Office.
